A brief assessment of any changes, some type of manual therapy(hands on to help joints and muscles move or work better), specific exercises to help stabilize and strengthen.

How Long Does A Physical Therapy Program Take To Complete?

Really depends on the diagnosis or the reason for coming to PT.  In general, 6-12 weeks.

How Often Are Physical Therapy Sessions Needed?

This also depends on the reason for coming to PT, it can range from 1 to 3 times per week, most people typically come in twice per week
